I consent to being photographed, interviewed and/or videotaped by representatives of Tainted Love, LLC, TLBK, LLC, Cann-I Cook and/or Bawlout Productions or any of their affilitates (henceforth collectively called “Tainted Love”). Any information or images obtained from those activities may be reproduced by Tainted Love for use in advertising, marketing, publicity and/or educational activities, including but not limited to Web sites, publications, videos, print and broadcast news. I authorize Tainted Love to copyright, use, and publish such photographs, video recordings, or audio recordings of me with or without my name and for any lawful purpose, including such purposes as publicity, illustration, advertising, and web content. I am 18 years of age or older and have read this release before signing, fully understanding the contents, meaning, and impact of this release. I understand that I am free to address any specific questions regarding this release by submitting those questions in writing prior to signing, and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release. I will be consulted about the use of the photographs or video recording for any purpose other than those listed above. There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed. I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against Tainted Love, LLC and its affiliate utilizing this material for promotional purposes.
I declare that I will not consent to having my image photographed or videographed by Tainted Love, LLC, TLBK, LLC, Cann-I Cook and/or Bawlout Productions, any of their affiliates or representatives (henceforth collectively called “Tainted Love”). I do not consent to use of my image or my name for any lawful purpose, including such purposes as marketing, education, publicity, illustration, advertising, and web content. Tainted Love does not control the actions or activities of other attendees. To ensure my image is not recorded by other attendees, I will state my desire to fellow attendees. To ensure my image is not mistakenly published by Tainted Love, I will send an image of myself as reference. Tainted Love will use due diligence to remove, blur and/or not publish my image in advertising, marketing, publicity and/or educational purposes but cannot guarantee without a submitted image as reference. I acknowledge that I have completely read and fully understand the above declaration and agree to be bound thereby.
Release of Liability Statement *
“In consideration for being allowed to participate in the Cann-I Cooking Classes (the “Activity”), I release from liability and waive my right to sue Tainted Love, LLC, TLBK, LLC, Cann-I Cook and/or Bawlout Productions, their affilitates, their employees, officers, volunteers and agents (collectively “Tainted Love”) from any and all claims, including claims of Tainted Love’s negligence, resulting in any physical injury, illness (including death) or economic loss I may suffer or which may result from my participation in this Activity, travel to and from the Activity (including air travel), or any events incidental to this Activity”. I am voluntarily participating in this Activity. I understand that there are risks associated with my participation in this Activity, such as physical and/or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability, death or economic loss. These injuries or outcomes may arise from my own or other’s actions, inactions, or negligence, or the condition of the Activity location (s) or facility (ies). Nonetheless, I assume all risks of my participation in this Activity, whether known or unknown to me, including travel to and from the Activity (including air travel) or any events incidental to this Activity.
